AAIB report on Dec 2024 incident: Air India Airbus A320 mistakenly attempted take-off from taxiway at Goa airport; lack of situational awareness cited

Digital Goa, August 17 – The Aircraft Accident Investigation Bureau’s(AAIB) final report on the 5 December 2024 incident at Manohar International Airport, Goa has stated that an Air India Airbus A320 began a take-off roll from a taxiway instead of runway 28 and was ordered to abort at about 123 knots; there were no injuries. Investigators blamed a deficit in situational awareness . The issuance of “Take-off clearance” by ATC likely caused the crew’s cognitive focus to shift entirely toward executing the Take-off. This shift may have diminished their situational awareness, resulting in expectation bias or cognitive overload, which prevented them from recognizing the visual cues indicating they were at the TWY intersection instead of RWY intersection, stated the report. The report recommended improvements in Electronic Flight Bag procedures, stricter cross-checks by crew, changes in air traffic procedures, and consideration of installing an Advanced Surface Movement Guidance and Control System at the airport.
